Something to chew on: Is smokeless tobacco more harmful than believed?

More than a quarter of a million people die each year from using smokeless tobacco, say scholars from the University of York, University of Edinburgh and Imperial College, London. Millions more have their lives shortened by ill health due to the effects of chewing tobacco-based products, they say, stating they are the first to assess the global impact of smokeless tobacco consumption on adults.

The team says governments and public health bodies need to consider incorporating the regulation of smokeless tobacco into policy frameworks.

Dr Kamran Siddiqi, senior lecturer in epidemiology and public health at the Department of Health Sciences/Hull York Medical School, said: “It is possible that these figures are underestimated and future studies may reveal that the impact is even bigger. We need a global effort to try and address and control smokeless tobacco.”

The study, which was funded by Leeds City Council and the Medical Research Council, estimates that in 2010 alone smokeless tobacco resulted in more than 62,000 deaths due to cancers of the mouth, pharynx and oesophagus and accounted for more than 200,00 deaths from heart disease.

Researchers compiled the figures using data from 113 countries and extracted from the 2010 Global Burden of Disease Study and surveys such as Global Adult Tobacco Survey. The results are published in BMC Medicine.

The team says that more research is needed in countries with high levels of consumption but where figures for the relative risk of acquiring smoking-related cancers are not available.

Dr Siddiqi said South-East Asia was a hotspot and in particular India which accounts for 74 per cent of the global disease burden.

He added: “Smokeless tobacco is used by almost a quarter of tobacco users and most of them live in South-East Asia. But even in this country there are communities in east London, Leicester, Birmingham, Leeds, Bradford and Birmingham using it.

“We have got no international policy on how to regulate the production, composition, sale, labelling, packaging and marketing of smokeless tobacco products.

“The international framework to control tobacco doesn’t seem to work to control smokeless tobacco. It doesn’t get the same regulation as cigarettes.

“There is a need to build on the insights obtained from efforts to reduce cigarette smoking and to investigate strategies to reduce the use of smokeless tobacco.”

Mr O’Reilly said there was a far higher percentage of potentially lethal illegal whites in Ireland than in Britain. He said there was some evidence to show some of these cigarettes smuggled into Ireland were later exported to Britain.

Mr Donohoe said the illegal whites were being smuggled into Ireland in containers through ports such as Dublin, Ringaskiddy, and Rosslare. It is believed the gangs are shipping in containers which each contain illegal cigarettes with a street value of €1m.

The profits are so lucrative that drug dealing gangs are increasingly turning to this illicit trade, especially as the penalties if caught are miniscule. A person found with more than €10,000 in drugs could face up to 14 years in jail. If it was €10,000 of illegal cigarettes or even more, they would probably get a fine. A jail sentence is extremely rare for cigarette smuggling.

The undercover team put all the contraband they get into evidence bags and the information they have gained during the course of their work, such as names, addresses, and phone numbers, will be passed onto the gardaí.

It is believed legitimate cigarette manufacturers, who have seen their profits tumble as a result of smuggling, will use evidence gathered by the team to lobby the Government for increased penalties for those caught with illicit cigarettes.

While they have a vested interest in a crackdown on such activity, they can also argue with some authority about the greater health risk of smoking illegal whites.

They will also point out the ease at which criminal gangs can make serious money to further their own activities and the knock-on cost this has to society, coupled with the huge loss of revenue to the State.

Cigarettes containing high levels of asbestos, lead, arsenic, traces of rat poison and human excrement are flooding Ireland as dissidents and highly organised crime gangs make vast profits buying them for as little as 20c a packet.(which is more than Big T makes them for).

The mark-up is huge, with a packet of 20 of what are known as ‘illegal whites’ selling on the streets for anything between €4 and €6.

The illegal whites are cigarettes with fictional brand names such as Excellence, Palace, President, CK, Gin, Ling, and M&G and are being manufactured in the United Arab Emirates, China, and Eastern Europe.

An operation carried out by former policemen in Cork in recent days showed the city awash with the illegal whites which were purchased from Asian, Irish, and Nigerian people. The team of four, which travelled from Britain, spent just a few hours walking around the city talking to people on the street before being able to carry out a number of purchases from different sellers, which included one shop in the city centre. The Irish Examiner accompanied the team on the operation.

They were led by former Garda Detective Chief Superintendent Kevin Donohoe and ex-Metropolitan Police Detective Chief Inspector Will O’Reilly. They have been working on behalf of Philip Morris International — which manufactures such brands as Marlboro and L&M — to get a snapshot of the rapidly growing illegal market.

Mr Donohoe said the team carried out test purchases in a number of other areas in the country in recent months and determined that 70%- 80% of all the illegal tobacco they have uncovered is counterfeit (hence 20-30% were genuine smuggled product showing the tobacco companies deliberately do not control their General Cargo supply chains)

and potentially very dangerous to people’s health (just like the real thing) . The remaining tobacco purchased was contraband — genuine brands of cigarettes and roll-your-own tobacco smuggled into the country without duty being paid. The former senior garda said Irish Customs believes the tax loss to the exchequer per year is around €250m, but Grant Thornton has put it as high as €500m.

The healthiest and easiest way to deal with addiction is to not become addicted, a simple truth that finally should become the foundation of law and policy regarding tobacco.

In March, the Institute of Medicine of the National Institutes of Health released a convincing study on how the nation could drive down nicotine addiction and save millions of lives, at no cost to the government. According to the IM, raising the legal age for tobacco purchases from 18 to 21 would result in 50,000 fewer deaths from lung cancer and 250,000 fewer premature deaths from all smoking-caused diseases for people born between 2000 and 2019. And, it would eliminate the health care and related costs that would accrue from those diseases.

It is well-documented that almost all smokers take up the habit and become addicted before they are 21. The IM study found that raising the legal age to 21 would reduce the overall smoking rate within the population by 12 percent, whereas raising the age to 19 would reduce the overall rate by only 3 percent.

The Food and Drug Administration commissioned the study but it does not have the authority to set a legal purchase age nationwide.

That is the purview of state and local governments. Four states have set the age at 19. New York City raised the age to 21 last year; Hawaii did so this year and California is expected to follow suit.

Needham, Massachusetts raised the age to 21 in 2005. According to a study in the journal Tobacco Control, smoking in Needham among those younger than 18 subsequently declined from 13 percent to just 7 percent, while smoking by people in that age group in nearby communities, where the legal age remained 18, declined only from 15 percent to 12 percent.

Although 18-year-olds are adults who can vote, join the military, drive, work at dangerous jobs and so on, they may not buy alcoholic beverages until they turn 21. Over the long haul, tobacco use is even more dangerous than drinking. Cigarettes and other tobacco products are the only consumer products that are lethal when used as intended by the manufacturers.

The state Legislature should begin the process of raising the legal age to 21.

NGO questions Japan Tobacco’s claim

TAIPEI, Taiwan — The John Tung Foundation (董氏基金會) has questioned Japan Tobacco Inc. (傑太日煙) about its claims of injecting over NT$9 billion in foreign capital into a tobacco plant being built in Greater Tainan.

In a press conference yesterday, the nongovernmental organization stated that “it is all a lie and the money is coming from Taiwan citizens’ pockets.”

Japan Tobacco raised the price on 21 kinds of Japanese cigarette brands on Wednesday by NT$5 to NT$10 per pack. The NGO said that the money earned by the price increase is meant to pay for the construction of a NT$9 billion tobacco plant.

“There was no foreign capital all along,” said John Tung Foundation CEO Yao Si-yuan (姚思遠). “In other words, Taiwan people are establishing tobacco plants that harm our own people.”

“Many people have complained that the health costs around cigarettes is growing gradually, yet the government has not adjusted taxes on cigarettes since 2009,” said Lin Ching-li (林清美), a member of the foundation, stating that the Japan Tobacco has raised prices several times, by NT$10 to NT$30 per pack each time.

While NGOs have demanded that the government stop the plant’s construction, the Greater Tainan government has claimed that it would help the local economy.

Moreover, the construction is already halfway completed and under the Investment Agreement between Taiwan and Japan (台日投資協議), local authorities cannot halt construction, the Greater Tainan government said.

The Ministry of Health and Welfare also stated that the cigarettes are for export and will not affect the health of people in Taiwan.

“However, there is no provision that the cigarettes produced can only be exported,” Yao said.

With over 35 percent of market share in Taiwan, the John Tung Foundation stated that about 700 million packs of Japan Tobacco products are sold in Taiwan per year. Through price hikes and the tariff saved due to the investment agreement, Japan Tobacco earns over NT$13 billion from Taiwan every year, “and this does not include other tax concessions, which cannot be counted.”

The foundation pointed out that the Investment Agreement between Taiwan and Japan is one-sided with regard to tobacco production. Japanese companies can invest in Taiwan while Japan remains closed to Taiwan, Yao said.

“Investment agreements between nations should be equal … (The inequality) shows that our government neglects the health of people and tobacco hazard prevention,” Yao said.

The NGO said that it hopes that the government will stop the construction immediately.

“According to the Investment Agreement between Taiwan and Japan, if there are more important public policies to achieve, the government can choose to expropriate (projects) under due process, if there is a legal public purpose and if there is compensation,” Yao said.

”There is a price to pay,” Yao added, “but if you are not willing to pay it, it would cost you much more in the future.”

Report titled ‘Smoky Truth’ about tobacco industry’s tactics launched

TheNetwork for Consumer Protection on Thursday launched a report “Smoky Truth” detailing how “Tobacco Industry in Pakistan carves room for Maneuver.”

The report in a book form was formally launched amidst the galaxy of health professionals including oncologists, policy makers, health rights activists, politicians and prominent members of academia and lawyers.

The report has been launched in the backdrop of a simmering controversy wherein the industry is using ‘non-health’ national and international actors against the health ministry to scuttle its plans to introduce strict regulations on tobacco industry to safeguard the health of the people of Pakistan as per fundamental right enshrined in the Constitution of Pakistan.

It was a rare occasion to see two recently diagnosed cancer patients, Muhammad Ziakat, 31 and Feroz Din, 55, launching the report.

Ziakat, a construction worker in Dubai returned to Murree, his homeland, last year on vacation. There he was diagnosed with cancer. So far, he has spent around Rs400,000 on cancer treatment. Feroz Din, a blacksmith from a small village of District Haripur, claims that he has been a smoker for the last 45 years and only quit it when diagnosed with CA buccal mucosa cancer. The deadly disease caused by smoking is already taking its toll as he has started disposing of his assets. Addressing the participants briefly, they said smoking had ruined their lives depriving them of all the joys of life.

Nadeem Iqbal, Executive Co-ordinator of TheNetwork, while briefing the audience about the report and various phases in its preparation, said, “Collecting information and compiling the report has been a huge challenge as the industry has ‘a number of skeletons in its closet’. This report is just a tip of the iceberg. There is much more deceit and gullibility the industry is employing to carry on with its nefarious business.”

Nadeem said that the report should be seen in the context of national and international tobacco control scene where industry is going into litigation against tobacco control regulations. The report such as Smoky Truth will help develop the legal arguments against the industry. The report also gives the message to the health community that they have successfully sensitised the patients to the harmful effects of smoking but now they have to tell everyone that its tobacco industry that is killing them.

He appealed to the doctors and lawyers that every cancer patient should be encouraged to sue the industry and claim damages from the industry. The lawyers should be providing free legal aid to such patients.

Dr Faisal Sultan, Chief Executive Officer, Shaukat Khanam Memorial Cancer Hospital, who had travelled all the way from Lahore to attend the report launching ceremony, said tobacco smoking was major cause of lung cancer. In his presentation titled ‘Smoking and Cancer’ Dr Faisal said a smoker is 17-time more under the risk of contracting lung cancer than a non-smoker. Besides cancer, smoking also causes heart diseases. He told the participants of the report launching ceremony that in the US it has been established that the quicker a smoker quits smoking the likelier are the chances of his survival than those of a smoking person.

Dr Javed Akram, CEO of PIMS and VC, Shaheed Zulfiqar Ali Bhutto Medical University, urged the need to discourage smoking at every forum. Terming the launch of the report a milestone, Dr Akram called upon the government to come forward and make strong regulatory framework to control its spread. He also asked the Ulema and religious scholars to issue fatwa against smoking as like any other drugs it is highly addicting. Cigarette is a symbol of ignorance and he/she who smokes should be discredited by us all, said Dr Javed Akram. He offered to conduct a study in response to the FBR’s statistics as what the country earns in form of tobacco taxes and what it loses in the shape of expenses spent on the treatment of cancer. Definitely health cost of cancer patients is much higher than the revenues collected from the tobacco industry, he said.

Human Rights activist Nasreen Azhar also condemned smoking saying she herself was a victim of passive smoking. “TheNetwork’s report serves as eye-opener for all of us and we need to give a collective message to the government to hold tobacco industry accountable for enticing the people, especially the youth,” she added.

Dr Shahzad Ali Khan from Health Services Academy (HAS) urged the need to ‘de-glamourise’ cigarettes and smoking. He said tobacco mafia was more powerful and more resourceful than tobacco control advocates.

Dr Aziz-ur-Rehman, Professor of Law at International Islamic University, (IIU) said Tobacco Industry was using litigation as a tool. He gave examples of Australia where tobacco control advocates had to wage a lengthy furious legal war against tobacco industry in order to adopt plain cigarette packs.

Dr Muhammad Faheem, Head of Oncology Department, Nuclear Medicine Oncology & Radiology Institute (NORI) in his presentation ‘Smoking a silent legal Assassin’ called cigarettes weapons of mass destruction. Every year, more lives are lost to tobacco and the challenge is becoming deadlier than the previous one, he said. If the trend is not checked, by 2020, tobacco will become the biggest cause of deaths around the world, he warned.

Mazhar Arif from Society for Alternative Media And Research (SAMAR) said smoking was not only a health challenge it was a very much political issue with its roots deep in the power corridors. Dr Vaqar Ahmed, Deputy Director, SDPI, termed TheNetwork report an authentic document telling us how Tobacco Industry is maneuvering to expand its business in the market.

2 fined S$31,000 for e-cigarette offences

An engineer and a property agent who imported and sold electronic cigarettes online were fined a total of $31,000 yesterday.

Francis Chue Kar Fatt, 33, was fined $16,000, while 32-year-old property agent Zhang Zhaoming, was fined $15,000 after they each pleaded guilty to 10 charges.

Two of Chue’s charges were for obstructing an authorised officer by deleting the www.thaivape.com website and a PayPal account which housed the evidence to all the e-cigarette sales transactions .

Another four were for selling and importing e-cigarettes with Zhang.

Following online surveillance of online electronic cigarette peddlers, Health Sciences Authority officers raided the Woodlands home of Chue and his wife, Ms Rattikan Khamtong, a 29-year-old Thai national, on Jan 7, 2013, for allegedly offering e-cigarettes for sale on the website.

They seized e-cigarettes, related peripherals and SingPost receipts.

Investigations showed that Ms Khamtong instructed her husband to delete the website while officers were conducting the search. She gave him her username and password and he did it from his office laptop at his office at Land Transport Authority at Sin Ming Drive.

On July 18, 2013, when HSA officers raided Zhang’s home in Sengkang, Chue deleted the PayPal account which contained e-cigarette sales transactions from the website.

Investigations showed that despite knowing it was an offence to deal in e-cigarettes, Zhang was still keen on the business. He started the e-cigarette online business with Chue’s help sometime in June. They agreed on the terms of the business and a profit-sharing arrangement.

The court heard that the goods were ordered online and payments made via Zhang’s credit card. The e-cigarettes were sold to customers at between $55 and $110 a set.

A warrant for the arrest of Ms Khamtong was issued last month.

Both men could have been fined up to $5,000 on each charge of selling or importing. For obstruction, Chue could have been fined up to $10,000 and/or jailed for up to 12 months per charge.
